01) File / New / 750 x 550 px / transparent

set your FGC to: #45215e (dark colour)
and your BGC to: #ebcbda (light colour)

flood fill with your dark FGC

Effects / Plugins / Graphics Plus / Cross Shadow: 
default settings

Effects / Plugins / Filter Factory Gallery N / Wee Scratches:
Amount: 255

02) open tube: be07.pspimage

Image / Mirror

Edit / Copy
close it again; we don't need it anymore
back to your picture 

 Edit / Paste as new layer

position as shown below:

03) Layers / New Raster Layer

flood fill with your light BGC

Effects / Plugins / Graphics Plus / Cross Shadow:

open: sg_embroidered-circle.jpg

minimize it; back to your picture

Layers / New Mask Layer / From Image:

Layers / Merge / Merge Group

close your minimized mask again; we don't need it anymore

Image / Resize: 55% (all layers NOT checked)

Image / Rotate / Free Rotate:
right / 90 degrees

position as shown below:

Effects / Plugins / Alien Skin Eye Candy 5: Impact / Extrude:
colour: #9055A9

Effects / 3D Effects / Drop Shadow:

Selections / Invert

activate top layer again

Edit / Cut (delete key)

Deselect

set opacity of this layer to: 75
